Baptism Certificate with Notations
Most often a Baptismal Certificate (dated within 6 months of your wedding) is needed before you can be married in the Catholic church. The certificate needs to be signed, dated, and embossed with the church’s seal.
Certificates will also be needed before a child prepares to receive a sacrament in a church other than the church of Baptism.
The following sacramental certificates might be needed if the sacraments were received at St. Louis or St. Valerie but never recorded at the church of your baptism.
